How to Prepare & What to Expect

  • Please make sure we have any pertinent medical records (including specialty appointments) or that you upload them to your pre-appointment form below. If you need help requesting records, please contact us at hello@brooklynyardvet.com

    Please fill out all of the appropriate forms and requested pre-appointment information by at least the night before your pet’s appointment. By completing these forms in a timely manner, you help the efficiency of your pet’s appointment. We want to dedicate the most time to what is really important - your pet and your questions!

  • Because of the high demand for veterinary appointments, we kindly ask that you confirm your appointment via our text reminder. Failure to confirm may result in forfeiture of appointment time as well as missed appointment fees.

  • Clients are welcome to accompany their pets in for their exam, however, we are still checking patients in curbside. Please call us at 503-505-9112 to let us know you are here and we will escort you and your pet in as soon as a room is available.

    Again, please make sure your pre-appointment forms are complete BEFORE your arrival.

  • Client Information Form

    If this is the first time visiting our facility, please fill out this form BEFORE your appointment. This form outlines our policies. We must have this form before treating your pet.

    If your contact information has changed, please resubmit this form.

  • Pre- Appointment History

    This appointment must be filled out for each pet before every appointment. This helps us to more efficiently gather history so our veterinarians can spend more time with you.

  • Behavior Consults

    If you are scheduled for a behavior consult, either in person or over the phone, please fill out this form at least 48 hours prior to your appointment time to allow for adequate review.

  • Health Certificates

    Please fill this form out if you are scheduled for a health certificate examination for interstate (including HI) and international travel AND have completed all necessary steps working with our designated staff member.

    Please have this form submitted at least 24 hours prior to your appointment.
